Application package for Postgraduate education

Application form for PhD program.

Application package includes:
- Application form (form attached);
- Scientific history (form attached);
- Resume (form attached);
- A certified copy of University graduate qualifications;
- A certified copy of University transcripts;
- A certified copy of Master graduate qualifications;
- A certified copy of Master transcripts;
- A certified copy of foreign language certificates that are still valid;
- A certified copy of other qualifications and certificates (if any).
Qualifications and certificates issued by foreign institutes have to be translated and certified and recognized by an authority in compliance with current regulations.
- Proofs of scientific research experience include 06 copies of published scientific research works (stapled in books, name written and list of titles in the cover page), in details:
+ Articles: a copy of the cover page, tables of contents, and the article contents.
+ Scientific researches: a copy of the Decision/Contract of conducting the research, the Minute of topic acceptance and the Summary of research topic contents (to those who are participants and are not the sole author there must be a confirmation of the research leader).
- 01 original and 05 copies of the Recommendation letter judging the applicant’s professional ethics, professional ability, and research ability (form attached).
- 06 copies of the overview of research intention of the applicant (form attached);
- 02 photos sized 3x4;
- Commitment of the study, research, and financial obligations for the education process as regulated (form attached);
- A copy of ID card;
- Health certificate.

Application package includes:
- Application form (form attached);
- Resume (form attached);
- A certified copy of University graduate qualifications;
- A certified copy of University transcripts;
- A certified copy of transferred transcripts (if any);
- A certified copy of other qualifications and certificates (if any).
- Confirmation from employer about one’s current work (in case the university major is different from the registered major);
- A certified copy of priority (if any);
- 04 photos sized 3x4;
- Health certificate (valid within 6 months);
- A certified copy of foreign language certificates that are still valid;
- A copy of ID card.
